Aerostar Down in South OZ

 July 7 
A twin-engine Aerostar crashed shortly after take-off at the Elliston airstrip on the west coast of South Australia

It appears the aircraft suffered a RH engine failure at or about lift-off resulting in the machine's inability to maintain height

"One engine has all bent-up propellers and the wing of it is actually damaged - the other one looks all right"

The pilot - the only person on board - is reported as receiving minor back injuries

CASA Issues Urgent AD on R-22's

 July 1 
CASA has found that the main rotor blade of the Robinson R-22 helicopter that crashed last week with the loss of the 2 persons on board - had failed

AD/R22/31 Amendment 4 now covers all rotor blades after 2 failures in the manufacture's "new and improved" blade

"The investigation has shown the main rotor blade failed..."

One Survived - Two Lost

 July 5 
A pilot of a hang glider has survived with broken limbs after one of the wings collapsed on his machine and his emergency parachute failed to open in time as he was too low

"...the fall was broken when his parachute caught in trees at Widgee Mountain west of Gympie"


Two persons lost their lives when the ultralight aircraft they were in impacted the ground near the town of Albury in NSW

Qantas Emergency at Sydney

 July 2 
Qantas flight QF6 had the right hand main wheel assembly of the Boeing B747 catch fire shortly after landing at Sydney airport

Emergency chute deployment evacuation procedures were ordered by the captain

"They yelled 'evacuate, evacuate, evacuate' then everyone leaped up from their seats ...and put out the slides"

Two evacuation chutes malfunctioned - some of the 250+ passengers received minor injuries
