The Senate Committee Report involving the Investigation on Aviation Accidents in OZ recommends that 26 changes be made after a highly critical examination of the ATSB report on the Pel-Air crash into the Pacific Ocean off Norfolk Island in November 2009 and the role and involvement of CASA in that report

The Report also recommended as a matter of urgency "..to implement the role of Inspector-General of Aviation Safety with the necessary powers, resources and expertise to oversee and independently review the activities of CASA the ATSB and other relevant organisations to an appropriate level"
